Rates and Insurance

Payment is to be made in full at the time of service. If you have a deductible that has not been met, you should be prepared to pay in full the day of your appointment. All copays will be collected at the time of service. If you need to make special arrangements, you can speak with the billing department to work out a suitable agreement.

Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. If you will be using private insurance, please call your insurance company to determine if you need pre-authorization. You are responsible for any fees not covered by your plan. There is an 800 number located on the back of your card, please check with your insurance carrier regarding your coverage by asking them the following questions-

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • What is the coverage per appointment?

  • Is referral required from my primary care physician?

**We accept all major insurance plans including Medicaid and Medicare**

Payment

Cash, check and all major credit cards accepted for payment.

**Late Appointment Policy**

In respect for any individual whom may be scheduled after your appointment, any person who presents tardy to their scheduled appointment by 10 minutes or greater, will be required to reschedule.

**Cancellation Policy**

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you may be required to pay a NO SHOW FEE. If you are unable to present to your scheduled appointment, the courtesy of a phone call to notify of this is kindly requested.
